Students fill out this form and obtain the needed signatures, and turn in at any time. Hours are to be completed by April 15, 2011.
Student Service for Transcript Credit
This form can be used to document service hours done above and beyond the required amount for your class. 50 hours earns .25 credits for a semester on the transcript and 100 hours earns .5 credits for the year.
