This form must be on file in the ALA office along with the indicated medications (prescription or OTC) in order for the office staff to dispense any medication. The ALA office does not provide OTC medications such as aspirin, ibuprofen, acetaminophen, etc.

 

ORDER YOUR ALA TRANSCRIPT


Current and former students must order transcripts via Parchment. Click on the link button below. A $3.50 Parchment fee will apply. 

Please keep in mind that your tuition account must be current. If it is not, the transcript fee cannot be returned to you. If you are not certain, you may contact the ALA Financial Office (602-268-8686 ext. 13 or Mueller@ALAcoyotes.org).
